Request Transcripts

To request that a copy of your official transcript be sent to a college university, employer or other authorized person, click on Request For Official Transcript, complete the form, and send it to Huntington College of Health Sciences (HCHS).  Only official written transcripts bearing the school seal will be issued. The first transcript is issued at no cost. Thereafter, a $5 transcript fee must accompany each request. A separate fee is required for each transcript recipient.
