To request that a copy of your official transcript be sent to college university, employer or other dully authorized person, click on Request For Official Transcript, complete the form, and send it to Huntington College of Health Sciences (HCHS).  Only official written transcripts bearing the school seal will be issued. The first transcript is issued at not cost. Thereafter, a $5 transcript fee must accompany each request. A separate fee is required for each transcript recipient. HCHS will not honor transcript requests of any student having a past due financial obligation to the college. Transcripts from other institutions found in admission files cannot be reproduced for student use. These transcripts must be obtained directly from the other institutions.
